1) The cable has a sort of rubber "block" about 6in from where it comes out the headset. This damn little block gets snagged in the sleeve on the top headstrap constantly when you fit it on, and thus it makes the top strap seem like it's adjusted too short. Always be mindful that the little rubber block slips past that damn sleeve and doesn't pinch the top strap shorter. Note I've also caught that little block getting snagged in the velcro overlap.
2) I have the hole in the strap's triangle aligned spot-on in the back of my head (I adjust that first) and the side straps going just above my ears, not quite over them.
I removed the little sleeve on the top strap of mine as it makes mounting then times easier, and hasn't affected cable management for better or worse. Once you find that good adjustment leave it, it should stick for as long as you need.
Do give the narrow facefoam a try, it's worth a shot.
